Does it compare the text or the appearance?
Both, separately. Text comparison finds wording changes even when the layout shifted; visual comparison catches a moved logo or a changed figure that the text does not mention.
What if the two files have different page counts?
Pages are matched by content rather than by position, so an inserted page shifts nothing else — it is reported as an insertion.
